Removing or changing MCP tools breaks agents that depend on them

7,128 of 15,360 reachable MCP servers (46.4%) have this issue today.

Why it matters

Agents store tool names and argument shapes. Removing a tool or renaming a parameter fails silently until a user notices. Prefer adding a new tool and deprecating the old one in its description for a release.
